APPLICATION FOR SERVICES

We’re happy to provide FREE Family Food Box and Bags of Hope meals, clothing, household items, and hygiene items for needy families in our area. But, to make sure we can help as many people as possible, we must require each person or family to qualify for services.

TO RECEIVE OUR SERVICES, WE NEED:

Proof of Rent

Either a receipt or rent contract showing the amount. If you do not rent in your home or are homeless please skip this step.

Proof of Income

One of the following: check stub, pension letters, SSI letter, unemployment verification, or bank statement showing direct deposit

Government ID

Please provide a photo of: State ID or driver’s license. If you have neither, City Mission staff will take a photo of you.

 Without the required components the application will not be processed.

Child identification

Each individual 18 years or younger living in your household must have identification in your application. One of the following: birth certificate, medical immunization card, or medical card.

This program is not sponsored by any government organization or run by the city. We are funded through the generous gifts of individuals who want to help low-income families, seniors, shut-ins, and the homeless.

If you qualify, you will be eligible every 30 days to receive help through our food, clothing, household, and hygiene programs. Should an emergency arise within 30 days, you may speak with the assistant director about your situation.
